TMS & TMSnat Simulation: free practice questions online
Your TMS simulation in the browser, aligned with the new German medical study aptitude test TMSnat: work through tasks, time yourself, get scored instantly and read the worked solution. No registration. Your answers are neither stored nor sent anywhere.
Choose a task group
How the simulation works
- Choose a task group and start with or without a time limit.
- Work through the questions without a calculator. Each question has five answer options, exactly one is correct.
- Score your answers: you see your correct answers and a worked solution for each question.
- Each round has as many questions as the respective group in the planned TMSnat, drawn at random from the group's question pool; for memorising facts, people and questions are generated anew each time. The order of the questions and answers is shuffled, so you can practise the same group several times.
Which task groups are available here?
You can currently practise all task groups of the planned TMSnat. We are continuing to expand the question pools.
- Arithmetic problem solving (Arithmetisches Problemlösen): 16 questions per round in 25 minutes (pool: 60).
- Quantitative and formal problems (Quantitative und formale Probleme): 20 questions per round in 50 minutes (pool: 60).
- Charts and tables (Diagramme und Tabellen): 20 questions per round in 50 minutes (pool: 40).
- Memorising facts (Fakten lernen): memorise 15 people in 6 minutes, then 20 questions in 7 minutes (generated anew each round). More on the page TMS memorising facts.
- Reading comprehension (Textverständnis): four texts with 20 questions in 50 minutes (pool: 8 texts). More on the page TMS reading comprehension.
- Basic understanding of medicine and natural sciences (Medizinisch-naturwissenschaftliches Grundverständnis): 20 questions per round in 50 minutes (pool: 40). More on the page TMS MedNat.
- Knowledge of natural sciences (Naturwissenschaftliche Kenntnisse): 40 questions per round in 60 minutes, 10 each from biology, chemistry, physics and maths (pool: 80). More on the page knowledge of natural sciences.
The number of questions and the time correspond to the TMSnat planning status (26 June 2026). Because the questions are self-written, this is only a guide to the pace, not to the difficulty. How the TMSnat is structured in detail is explained on the page TMSnat overview.
TMS or TMSnat: what are you practising here?
Many people still search for a “TMS simulation”, even though the test is currently being changed. According to tms-info.org, the autumn round of 2026 is the last TMS in its previous form. From 2027, the TMSnat is expected to take its place. What matters for registration is on the page TMS registration. The practice on this page is therefore aligned with the planned task groups of the TMSnat.
Many people already know the task group Quantitative and formal problems (Quantitative und formale Probleme) from the previous TMS. According to the planning status, it is also part of the TMSnat, with 20 tasks in 50 minutes. Arithmetic problem solving (Arithmetisches Problemlösen) is planned in the TMSnat with 16 tasks in 25 minutes. According to the current planning status, calculating under time pressure is therefore a fixed part of the TMSnat. Charts and tables (Diagramme und Tabellen) is also planned as a group with 20 tasks in 50 minutes. How to prepare for all eight task groups is explained on the page TMSnat preparation.
What to expect in the individual task groups
Arithmetic problem solving
Short word problems with a medical background: prices and percentages, rule of three, mixtures of solutions, infusion and drip rates, work rate and speed. The calculations themselves are usually not hard. The difficulty is finding the right approach in about one and a half minutes per task without making a slip.
Quantitative and formal problems
Here you are given a relationship, such as a formula, a conversion or a law of growth or decay, and apply it to a new situation. Typical tasks are converting units, recognising proportionalities (what happens to the result if one quantity doubles?) and half-lives. According to the planning status, you have about two and a half minutes per task on average.
Charts and tables
You are given a table or chart with invented data and evaluate it: reading values, comparing, calculating shares and changes. In each question exactly one of the statements is true. According to the planning status, you have about two and a half minutes per task on average.
Memorising facts
You memorise case descriptions of 15 fictitious people: name, age group, occupation, one characteristic and the diagnosis. The list is then hidden and you answer 20 questions about it, without notes. Strategies and background are on the page TMS memorising facts.
Reading comprehension
You read factual texts from medicine and the natural sciences and check which statements can be derived from them, often in combination tasks with statements I to III. You need no specialist knowledge; only the text counts. More on the page TMS reading comprehension.
Basic understanding of medicine and natural sciences
Each task brings a short description from medicine or the natural sciences, for example about a hormone, a mode of inheritance or a physical rule. You check what can be derived from it, sometimes with a small calculation. More on the page TMS MedNat.
Knowledge of natural sciences
Here real subject knowledge is needed: biology, chemistry, physics and maths at upper-secondary level. No topic catalogue has yet been published for the TMSnat; our questions follow the official HAM-Nat topic catalogue. More on the page knowledge of natural sciences.
Tips for practising
- Approach first, then calculate: Briefly note which quantity you are looking for and which units are involved. Many mistakes come from mixed-up units, not from wrong arithmetic.
- Estimate instead of calculating exactly: In many tasks the answers are far enough apart that a rough estimate is enough to spot the right one.
- Don't get stuck: If you still have no approach after about a minute and a half (arithmetic problem solving) or two and a half minutes (quantitative and formal problems, charts and tables), move on to the next task and come back at the end.
- Read the solutions even for correct answers: There is often a shorter way than your own. That is exactly what saves time in the test.
- Practise with a time limit: Without time pressure, many people solve almost every task. The difference only shows with the clock running, so practise with the timer regularly.
- Train mental arithmetic: The practice here is meant to be done without a calculator. Which aids are allowed in the TMSnat is stated in the information from the test providers.
Frequently asked questions
Are these original TMSnat questions?
No. No TMSnat has been published yet. All questions here are self-written practice questions. They are meant to give you a feel for the task types and time pressure, but they do not replace original materials.
Are my answers stored?
No. Scoring runs in your browser. When you leave or reload the page, your answers are gone. We set no cookies and load no content from third parties. How the server processes technical connection data when you open a page is described in the privacy policy (in German).
How are the results scored?
We only count the correct answers. There is no conversion into official TMSnat points or percentile ranks, because no official information is available for that. How the previous TMS was scored is explained on the page TMS result.
Where can I find official information and sample questions?
The test providers publish current information on registration and procedure on tms-info.org. You will also find official information material on the previous TMS there. We do not imitate the providers' original material here.
When will more questions be added?
We are expanding the practice step by step. We deliberately do not give a fixed date. It is best to check back from time to time.
